How to Join

Join The CLUB for only $15 for the entire year!

Download the Member Registration Form – Membership is for ages 6 to 17 years only.
Member Application Form (English/Spanish)   WORD Fill in Form To qualify for FREE membership you must be a Pomona resident and bring the following items:
Club Membership is open to children ages 6 to 17 years.  No matter where you live, just follow these simple steps:

  1. Complete a Membership Application
  2. Get Parents or Legal Guardian signature
  3. FREE Membership Requests (or $15 per child for 1 year membership):
    1. ADDRESS:  Bring COPY of a bill, report card, etc. that shows your address
    2. INCOME:  Bring COPY of Proof of Income (check stub, general assistance income, etc.  If unemployed contact us for details.)
    3. If a child is 6 years old a COPY of the Birth Certificate is required.
  4. Last, bring ALL documents and payments to the mandatory parent meetings every 1st and 3rd Wednesday of each month at 5:30pm!

Membrecia del Club es disponible para niños entre 6 y 17 años. No importa donde viva, simplemente siga las siguientes instrucciones para aplicar:

  1. Llene la Aplicacion de Miembro
  2. Consiga la firma de los Padres o Guardian Legal
  3. Puede calificar para Membrecia GRATUITA (o $15 por cada niño(a) por todo un año):
    1. DOMICILIO: Traiga una COPIA de un bil,  boleta de calificaciones, etc. que compruebe su domicilio.
    2. INGRESOS: Traiga una COPIA de comprobante de ingresos (su ultimo cheque, asistencia monetaria, etc. Si esta sin trabajo llamenos para mas detalles.)
    3. Si su niño(a) tiene 6 años, es necesario que traigan una COPIA de su Acta de Nacimiento.
  4. Por ultimo, traiga todos sus documentos y pago a la junta obligaoria para padres. Son el 1er y 3er Miercoles de cada mes a las 5:30pm!
